Progress Report
Use this form ONLY IF THE PROJECT WILL NOT BE COMPLETED by July 1st, 2011. If the project wil be completed by or prior to July 1st, 2011, use the FINAL REPORT FORM. A Progress Report or a Final Report form must be received by August 2, 2011.
Instructions for Completing online Progress Report
The report is available as an online accessible word document. Please make sure to save a copy of the report to your computer by clicking file under save as. You now have the access to type within the form and save it to your hardrive as you complete it. Please note that after completion, you must print the report and mail or deliver to HPCVB with the applicant's signature. Please do not alter the format, wording, or texts of the application. 2010/2011 Online Progress Report
Final Report
If the project will be completed by or prior to July 1, 2011, then use this form. Otherwise, submit the Progress Report by July 31, 2011 and submit the Final Report by December 31, 2011. A Progress Report or a Final Report form must be received by August 2, 2011.
